What insurances does BOT accept?
Medicare: We are accepting Medicare Primary + Most Supplemental Insurances
Blue Cross Blue Shield Highmark: Starting May 2025, we are no longer in-network with BCBS. We provide a superbill to you, and you are able to submit for reimbursement of these services up to their usual and customary rate according to your out-of-network benefits.
Independent Health: Most Commercial plans and Medicare Advantage Plans

Starting October 1, 2025, Independent Health Medicare plans will require prior authorization for therapy.
This means we must get approval from your plan before you can use your therapy benefits, which may add some delay.If another clinic couldn’t get your visits approved, it doesn’t mean we won’t.
Approvals depend on your medical needs and how clearly your therapist explains the treatment plan using assessments and evidence-based care. We’ll submit everything needed and keep you updated.What you can do:

Straightforward Out-of-Pocket Costs
90 Minute Comprehensive Evaluation: $250.00
60-Minute Treatment: $105.00
A superbill can be provided for you so that you can submit this to your insurance carrier or health savings account company for reimbursement.
Can I be reimbursed by my insurance carrier if I pay out of pocket?
You can contact the insurance company independently to see if they can reimburse you for some of the services. We can provide a Superbill and receipt. Buffalo Occupational Therapy is no longer calling/contacting/acting as a liason between patient and their insurance companies.
Late Cancelation / No Show Fee
If you are 15 minutes late or more there is a $50 charge
If you cancel for any reason within 24 hours, this is treated as a no show and there is a fee of $50
